Applying for a Disability Benefit

1. Forms:
   • Application for Disability Benefit
   • TD1 – Personal Tax Credit Return
   • Direct Deposit Information
   • Appointment of Nominee
2. Documentation:
   • A notarized copy of Teacher’s Birth Certificate

Upon receipt of the application forms and supporting documentation, the benefits will be calculated in accordance with the rules set out under The Saskatchewan Teachers Superannuation Plan. The benefit will be paid on the last banking day of each month.
